2. Selection Memo from the CEE/DME/DGHS, New Delhi
3. 10th Certificate/Matriculation
4. Community Certificate (for SC/ST and Educationally Backward Classes students having
annual income below 2.5 lakhs
5. Income Certificate (for Backward Class students, if selected as such)
6. Pass Certificate of the Qualifying examination
7. Mark list of the qualifying examination
8. Transfer Certificate
9. Conduct Certificate
10. Passport size photographs (2 copies)
11. Bond in the prescribed format in Kerala Stamp Paper worth Rs.100/-
12. Certificate of vaccination of HBV, MMR and Chickenpox
13. Declaration signed both by the student and guardian
